Hi, I'm Tiara
I'm a Board-Certified Patient Advocate (BCPA), Community Health Worker, and chronic disease educator. I've spent my career in the space between what healthcare offers and what people are actually able to receive.
I also know that gap from the other side of the chart. I'm a heart transplant survivor. I've been the patient holding the paperwork, asking the questions, and trying to figure out what happens next.
That combination of my credentials and my lived experience is what drives the passion behind the work that I do.

The Five Domains
Social Determinates of Health
Most of what determines your health happens before you ever see a doctor. Researchers call them social determinants of health. In plain terms: the conditions you live in, work in, and were born into — and they account for far more of your health outcome than the care you receive.
Four of the five have nothing to do with medicine.
ECONOMIC STABILITY
Income, employment, debt, and whether the math works. Medical bills, prescription costs, insurance premiums, and the impossible choice between a copay and groceries. When money is the barrier, no treatment plan survives contact with real life.
EDUCATION ACCESS & QUALITY
Health literacy, plain-language information, and whether anyone ever explained things in a way you could use. Only about 12% of American adults have proficient health literacy — and that holds across education levels. Confusion isn't a personal failing. It's a design choice.
SOCIAL & COMMUNITY CONTEXT
Support systems, isolation, caregiver strain, discrimination, and trust. Whether you have someone who shows up. Whether the health system has earned your confidence — and in a lot of communities, it hasn't, for reasons that are historical and documented.
NEIGHBORHOOD & ENVIRONMENT
Housing conditions, air quality, food access, transportation, safety, and heat exposure. Whether there's a grocery store or only a gas station. Whether there's a bus that reaches the clinic. This is the block your health starts on.
HEALTH CARE ACCESS & QUALITY
Having coverage, having a provider, being able to get there, and being taken seriously once you arrive. Access isn't just whether a clinic exists. It's whether it's open when you're off work, whether it takes your insurance, and whether the last visit made you want to come back.
